Call the Doctor

I'm starting to think that I should be making a 2nd call to our doctor. I called them a few days ago and got an appointment set up for us to get our adoption physicals. However, now that I'm thinking about it, she might not be completely aware of all that entails.

I'm looking at the form we got from our agency... they have a big ole list of what needs to be checked. They are...

Hearing

Vision

Heart

Liver

Lung

Thyroid

Nervous System

Lymphatic System

Routine Blood Test

Liver Function

Routine Urine Test

HIV Test

Hepatitis B

TB

Sounds fun, eh? Something tells me we're gonna have to fast for this one, and they didn't mention anything about that... Yah, i think i have to call them back.

Not only did they give us this list, but they have a whole section with Instructions for the Examining Physician!

Instructions to the Examining Physician:

If the patient answers “yes” or “abnormal” to any questions or is on any medication, a typed explanation on the doctor‟s letterhead is required and should contain the following: Here's hoping everything comes back normal! Then we won't have to worry about any of this.

1) Please do not use any abbreviations or medical terminology; keep in mind that the letter will need to be translated.

2) List and explain the illness/condition.

3) Summarize the patient‟s history with the illness/condition.

4) Outline the patient‟s treatment, both historically and ongoing.

5) Outline the patient‟s recovery and current health status.

6) Provide a prognosis for the individual‟s likelihood for continued health.

7) Provide a recommendation that the individual would be able to adequately care for an adopted child.

8) For Medication, please state the name, dosage, and purpose of the medication; and the reason & length of time that the patient has been taking the medication.
